Guidelines for filling the Registration Form
General Information
1. Age : 3+ as on 31 st March 2012 (For Pre School) , 4+ as on 31 st March 2012 (For Pre Primaryl)
2 . Application forms are available at the school reception counter from 2nd January 2012 to 16th January 2012 during school hours.
3 . The following schedule will be observed for Nursery (Pre School) Admissions 2012-2013

Instructions for Filling up the Registration form:
• The form should be filled in by the parent in their own handwriting.
• Please read the instructions carefully before filling in the form to avoid rejection of your form.
• Please fill in each entry of the form and write N.A for those entries, which are not applicable to you. Do not leave any entry blank.
• The Registration Formality will be complete only on the receipt of the duly filled registration form.
• Incomplete forms will not be accepted.
• Please enclose the following required documents along with the registration form
• Attested copy of Date of Birth Certificate of the child
• Attested copy of Proof of Residence-Voter Identity Card/ Ration Card/ Passport
• Proof of Transfer (if applicable)
• Proof of Sibling / Alumni (if applicable)-photocopy of the I-card / 12 th class Mark-sheet.
• For EWS –copy of Ration Card and Certificate from SDM stating annual income.
• For SC / ST / OBC, attach relevant attested documents.
• Attach a copy of the relevant document, if the child has special medical needs.
